Medication review and education

Medication reviews allow you to stay informed on the medicines you’re taking and how they interact to provide a more holistic, individualised health assessment.

This service aims to provide you with education on your existing prescription(s) and how they interact within your body so you feel confident in making informed decisions and advocating for your health.

Acumend provides one-on-one sessions with a registered pharmacist who thoroughly reviews your existing medication(s) and takes time to explain how they function in your body, answering any questions you may have.

The aim of medication reviews and consultation is to maximise an individual patient’s benefit from their medication regimen and prevent medication-related problems.

Our registered consultant pharmacist also offers additional general health checks including:

  • Diabetes medication reviews
  • Chronic Obstructive Pulmonary Disease (COPD) screening
  • Asthma device technique check
  • Clinical interventions
  • Adverse Event reporting
  • Blood glucose, blood pressure and cholesterol checks

Note: Along with private medication reviews, Acumend can issue leave certificates (up to two days).


Private medication reviews

A private medication review is provided by a Consultant Pharmacist to assess and evaluate your medication regimen. It is to ensure that the medication you are taking is safe, effective, and appropriate for their specific health needs.

Our process involves a patient interview, a clinical assessment, and a written report that can be provided to your primary care physician or specialist.

We will evaluate the appropriateness of each medication, potential drug interactions, and side effects, and provide recommendations for medication changes or adjustments, as well as lifestyle changes that can improve your overall health outcomes.

Private medication reviews are particularly helpful for individual who are taking multiple medications, or have complex health conditions.


Medication education sessions

The medication education sessions can be conducted either in-person or online, depending on your preference. Before the session, you will be asked to submit a list of your current medications to the Pharmacist Consultant.

During the 30-minute consultation, the consultant will provide education on your medications and answer any questions or concerns you may have. This session can help you feel more confident and informed about your medication regimen.
